It's even more mad when you're the patient! My brother-in-law has undergone something like this a few times to have his bone marrow checked for malignant cancer cells. Straight through the breast-bone, I imagine the needle is bigger though. He passed out the first time, now he requests a general anaethestic for the procedure.
This is pretty close to how I picture it.
