If you photograph the same model several times, with the same conditions could you have only one model release or do you need one for each session?

If it's the same shuld it states anything regarding beeing used for several sessions?

How do you do it?

One release form for each shoot/session :)

Yes, you can use the same one. Say you plan to shoot with a model that will take several days, you can use the same model release. Just put in the days you will shoot with the model.

I've used blanket dates for people I shoot a lot of for stock. Usually I date them for a full year. Never had a problem.

Model releases are to protect both the photographer and the model. If for any reason you are taken to court over an image you'll want a MR that clearly relates to a specific shoot. A blanket/catch all release is very unlikely to satisfy the judge.
It might seem like a pain, but it will be time well spent.

If it is for stock photography it TOTALLY depends on the site.

iStock REQUIRES that you have the model sign the release on the same day the photos were taken (re: make sure your camera settings are correct and your model doesn't screw up the date on the release (like putting their birthday in the shoot date field)

Dreamstime prefers / requires that you ONLY have ONE release per model. So even if you shoot a model several times over several years, they want just one release.

what I would do: Get a release for every shoot, it isn't that much work and would protect you MUCH better if you ever needed it in court. Send each release to iStock and just send one of them to Dreamstime.

Do you use your own release form, or do you use the ones available from dreamstime, Shutterstock, etc.?
I am new to this and I have not taken pictures with people yet (at least not ones that I might submit for stock). Do you decide in advance which site you will submit to and use that site's release? or do you have the model sign releases for any/all of the sites you contribute to so you can decide later?
I'm so worried about getting the releases right, that I am avoiding taking pictures of people!

I don't shoot model very often, but I use my own model release form and had so far no problems with any agencies. If you do create your own, look through the form of the different agencies, and make sure that all the information that they request on there forms are included.

Also iStock requires now that you have the model sign a new model release for every shoot event.

Thank you for your advice. I will probably make my own. Some of the ones from the agencies have typos! It might make me a bit crazy to have people signing a form that has "Dat" instead of Date. :)
