what exactly do people mean by that and how if one wanted a "hard" image would be the way to go about it?

Soft focus is one way to get a soft image and crisp focus would, of course, be the opposite...although I've never heard it referred to as a "hard" image.

A "hard" image would be getting a shot of my son smiling. It's not that he is unhappy, he just won't smile when the camera is pointed at him.

In a bad way, people can call your image soft if it isn't sharpened properly (not enough sharpening). The focus tends to be soft (not to be confused with intentional soft focus).

Ever watch the original Star Trek series? Every closeup of a woman is in soft focus, especially those who Kirk was womanizing. ;\
