Selective desat? Just as a thought, try changing the color on the pills- something more vibrant to contrast with the black and white (pink?). The lighting makes for a neat abstract, but I'm sort of thrown off by the "ew, medicine"-ness of the subject matter. Just one girl's opinion :-)

Interesting take, although I thought they were cocktail swords from the thumbnail ;-). The shadow seems a little overpowering- maybe a lighter or darker background to offset the subject?

You might want to try adding noise by using the monochromatic feature- it might look more like film grain instead of digital noise.
